HEAL Wellness Center Liability Waiver

I have read & received a copy of the Notice Designed to Comply with the State of CA Guidelines in The Business and Professions of the State of California Section 2053.6. 

If you are a Federal, State, or Local Agent, upon entering these premises you must declare the same or under the Bivens Act, Article 42, be held personally and individually liable. I have read this statement and declare that I am not an agent. 

I take full responsibility for any services, products, add-ons, implants, and/ or supplements I choose to try to assist my health before, during, and/ or after my session, including, but not limited to the complimentary use of the amethyst crystal biomat and any water I consume with added electrolytes or minerals. I understand that HEAL Wellness Center and the practitioner(s) is not a physician or pharmaceutical representative and thus cannot speak to the efficacy of such supplements or complementary treatment services. I have conducted my own due diligence about offered add-ons and implants and obtained a prescription (if pertinent) from my physician for any add-ons or implants. I do not hold the practitioner(s) or HEAL Wellness Center accountable for any reactions or detox symptoms I may have to any services rendered and/ or aforementioned additions or add-ons. 

I understand that for any implants or add-ons that were requested during time of service, that an invoice will be sent to me after my session. I understand that if I fail to pay such invoice within one week of date of service, that my credit card on file will be charged. I understand that for any outstanding invoices on my account, that my credit card on file will be charged after given proper notice (phone & email contact) to respond to billed invoices. 

I understand that a 48-hour notice is required for all changes/ cancellations to my appointment(s). If I booked online and changed or canceled outside of that time-frame, I understand that I forfeit payment. If I booked in person or over the phone, and changed or canceled outside of that time-frame, I understand that my card on file will be charged for the full cost of scheduled services. I understand that if I have a package, and changed or canceled outside of that time-frame, a credit from that package will be forfeited. If I have a membership, and changed or canceled outside of that time-frame, a late fee for the full cost of the service(s) will be charged to my credit card on file.  

I understand that all special discounted programs, packages, and memberships are non-refundable and non-transferable.
